Purpose: to evaluate the changes in higher order aberrations (HOAs) after Wavefront-optimized (WFO) LASIK for the correction of myopia and astigmatic myopia.

Patients and methods: Non-comparative and prospective study. WFO LASIK on Allegretto Wave Eye-Q system was used to correct myopia and astigmatic myopia for 33 patients (66 eyes). Main outcomes were UCVA, BCVA, refraction, higher order aberrations measured with the Allegro analyser (Tscherning type) at preop, 1 week, 1 month and 3 months postoperatively.

Results: The mean age of the patients was 24±4.4 (18-38). The mean spherical equivalent was -4.98±2.04D (-1.5 to -11.25D) preopratively, 0.05±0.27 D at 1 month and -0.01±0.21D at 3 month postoperatively. Fifty-seven eyes (86.4%) achieved UCVA of 1.0 and better and 100% eyes achieved ±0.5D of attempted refractive correction after 3 months. The total root-mean-square increased 1.98 times and spherical aberration increased 2.94 times posoperatively and these induced changes did not correlate with the corrected refraction.

Conclusion: WFO LASIK induced significant increase in higher order aberrations despite excellent visual outcome.
