Is that possible performing phacoemulsification in cheap situation
at the suburb area on the developing country?

Objective :
Saruni Eye Clinic is a suburb clinic at the Pandeglang-Banten, 125 km far from Jakarta the capital city of Indonesia. We know that cataract handling by phacoemulsification is still very expensive surgery for most of people on the developing country.

Methods :
One hundreds and fiftenn eyes from 84 payable cataract patient and cross-subsidized for 31 non payable patient were operated by performing phacoemulsification with IOL implantation. Two surgeons were noted. Data were collected from medical record since July 2009 until October 2010. Surgery were using the diplomax one phacomachine and appasamy operating microscope. Foldable biconvex acrylic optic spheric and aspheric three piece IOL for payable and PMMA IOL for non payable were implanted. Consumable using the HPMC I.P. 2% OVD, Ringer-Lactat solution with gentamycin injection, tryphan blue 0.6mg/ml, Carbachol 0.01% and xylocain for topical-intracameral.

Results :
The average of age is 50,58 year-old (40-87 yo). Male mostly (54%). The density of cataract is grade IV(41,67%), grade V (9,52%) and grade I (2,38%). The visual acuity on pre operative was about Light Perception-Hand Movement (51,19%) and post operative is 0.9-1.0 (39.28%) on day one. Three patients found with retinal diseases. Three patient is not implanted the IOL.

Conclusion :
The simple devices for phacoemulsification technique with premium IOL implant can give the best result for the cataract patient on the suburb. This is not expensive surgery and possible to make everybody "see" eventhough for the poor patients.
