Purpose: To analyze outcome predictors for curative effectiveness of therapeutic keratoplasty.

Methods: Hospital chart of 100 consecutive eyes that underwent therapeutic keratoplasty was reviewed to identify causative organism(s), mode of presentation, and the effectiveness of the procedure evaluated in terms of infectious cure, graft clarity, anatomical success, follow up rate and the outcome predictors identified

Results: Therapeutic cure was achieved in 86 eyes, 14 eyes developed re-infection out of which seven needed evisceration. Sixty percent of graft failure was noted by one year. Outcome was influenced by infiltrate size, perforation, timing of surgery and follow-up compliance.

Conclusion: Therapeutic keratoplasty treat refractory infectious keratitis with high cure rates. Outcome predictors provide guidelines for optimizing outcome and patient counseling.
