Heavy Silicone Oil Tamponade for Retinal Detachment with Macular Hole

Purpose: To study the efficacy of heavy silicone oil (HSO) tamponade for the treatment of retinal detachment with macular hole.

Methods: Twenty eyes of 20 patients were included in this retrospective study. The inclusion criteria were retinal detachment with macular hole. Vitrectomy, inner-limiting membrane peeling and HSO tamponade were performed in all the patients. Other procedures, such as epiretinal membrane peeling, retinotomy, endo-laser photocoagulation, phacoemulsification and intraocular lens implantation were performed as necessary. No special posture was required postoperatively.

Results: All the patients were high myopic and had choroidal atrophy at the posterior pole. Four (20%) cases were retinal redetachment after vitrectomy. Six (30%) patients had choroidal detachment. The follow-up time was 3-14 months ( 7.3±3.8 months). Retina reattachment was achieved in 18 (90%) patients with one surgery, and in 19(95%)patients with a second surgery. One patient still had localized RD after a second surgery. The visual acuity improved from mean logMar 2.77±0.81 pre-operatively to logMar 1.22±0.46 post-operatively (p<0.00001). All the phakic patients developed different degrees of cataract, mostly sub-capsular cataract. The pre-operative IOP was 11.1±10.2mmHg and the post-operative IOP was 13.9±9.6mmHg(p=0.10).

Conclusion: HSO endotamponade is a safe and effective treatment for retinal detachment with macular hole.
