Purpose: To illustrate the anterior segment optical coherent tomography images in eyes with postoperative capsular distension syndrome and the management.

Methods: Prospective interventional case series study.

Results: ASOCT scans clearly reviewed distension of posterior capsular bags in six patients who got uneventful phacoemulsification and intraocular lens implantation. Scans were performed few weeks (early) to few years (late) after the cataract extraction and with the clinical suspicion of capsular block syndrome. Conditions were managed with either laser capsulotomy according to the degree of the capsular bag distension assessed by ASOCT.

Conclusions: ASOCT provides a non-invasive and more objective assessment of the posterior capsule distension after the cataract operations. The images were useful in the diagnosis and management of postoperative capsular block syndrome.
