The Diagnosis and Therapy of Devic's Disease

Purpose: To report a case of Devic's disease or Neuromyelitis optica (NMO) in a 19-year-old Indonesian woman.
Methods: Case report. A 19-years old woman suffering decreased vision in both eyes recurrent since one year ago. There were history leg and arm paralysis. No history of systemic or hereditary diseases. Visual acuity on right eye was no light perception, and left eye was hand movement. The anterior segment, showed iris and pupil were round, mid dilated, without light reflex in the right eye. The left eye, iris and pupil were round, mid dilated and decrease light reflex. Fundus showed optic atrofi in the right eye, and slighty pale in the left eye. The magnetic resonance imaging of brain showed no abnormality, but on spinal-cord revealed intramedullar lesion. We made the diagnosis of Devic's disease with differential diagnosis of Multiple Sclerosis.
Results: The diagnosis will be absolute if the patient meets 3 criteria required and supportive diagnosis if there are either 1 major or 2 minor criteria required. Five days after injection of dexamethasone 1x30 mg, there was improvement of the visual acuity in the left eye to 4/60.
Conclusions: In this case, we made the diagnosis of Devic's disease based on 3 absolut and 2 major criteria. Injection dexamethasone was successful in preventing the progression of the disease
