Traumatic Ocular Injury with Double Perforation

Purpose: To report management of Traumatic Ocular Injury with Double Perforation caused by screw machine

Method: This is a case report study

Results: A 20 years old man came to Cicendo Eye Hospital after accident from screw machine which penetrate the left temporal bone to the ocular sinistra. The visual acuity of the left eye was hand movement. On Skull 3D CT Scan examination reveals a liniear hyperdense consider as a nail penetrated from temporal bone and stucked on left orbital rim..Two step procedures were planned in this case. During expolaration through lateral orbitotomy the surgeon found the screw was penetrate from left temporal bone to inferomedial ocular sinistra resulted sclera rupture, the foreign body was removed and scleral suture performed. Second procedure which was Pars plana vitrectomy combined with endolaser photocoagulation and SF6 injection was done a month after. Three months post operatively the visual acuity was counting finger at 3 meters with retinal fibrosis at temporal arcade of posterior pole on left eye.

Conclusion
Management of ocular injury with double perforation had a succesed result with lateral orbitotomy surgery and was a safe technique to extraction foreign body in the orbita
