Spontaneous Closure Of A Traumatic Full Thickness Macula Hole

Purpose: To report a case of spontaneous closure of a full thickness macula hole.

Methods: A 37 year old Indian male was referred after a motor vehicle accident, complaining of blurring of vision. Upon examination, his vision was 1/60 with no relative afferent papillary defect. Anterior segment showed iritis. On fundus examination, patient had left macula hole with positive Watzke-Allen test. OCT was done and it confirmed the diagnosis. He was started on topical steroids for iritis and topical NSAIDS (Nevanac).

Results: 1 week later patient's vision improved to 6/60 and repeat OCT showed resolving macula hole. Patient was seen again after 6/52 and this time there were no more macula hole seen on OCT. His vision improved to 6/9.

Conclusion: Ocular contusion or trauma can lead to complications such as macula hole due to immediate rupture of the macula at its thinnest point. These holes can resolve spontaneously without any surgical intervention as in this case.
