Bilateral Peripheral Retinal Capillary Hemangioma: a Case Report and Review of Its Management

Pupose
To report a case of bilateral retinal capillary haemangioma and to review the clinical management options
Methods
Twenty-five years old Indonesian man visited Vitreoretinal Division, Departement of Ophthalmology, Cipto Mangunkusumo Hospital with a chief complaint of gradual blurred vision of both eyes. He had no history of having tumor in his family. No systemic or central nervous system lesion was found. Visual acuity was 6/30 in both eyes. In funduscopic examination 1.5 disc diameter orange-red-spherical tumor located superotemporally in both eyes was found. Dilatation of feeding artery and draining vein were found. The patient was treated by photocoagulation. One month after photocoagulation, visual acuity of his right eye improved to 6/15 and became 6/30 three months later. Visual acuity of his left eye improved to 6/5 one month after photocoagulation and remain the same three months later.
Result
Four years later, the visual acuity of his left eye was half-meter finger counting; while visual acuity of the right eye was hand movement. Subretinal fluid was found in both eyes. Although the size of the lesion was relatively stable, but fibrosis and retinal exudate were developed in the left eye. After reviewing the management options, cryotherapy was the best further treatment.
Conclusion
Bilateral retinal capillary hemangioma has a poor visual prognosis despite the given treatment and the best option for further management in this case is cryotherapy.
