Tractional Retina Detachment in High Myopic Eyes

Purpose: To evaluate clinical characteristics and surgical results of vitrectomy in managing high myopic eyes with TRD.

Methods: Retrospectively reviewed 12 cases of TRD with high myopia. F to M ratio 8:4. Age from 48 to74. Refraction error -10 to -29D; axial length 28-34mm. Preoperative V.A. between HM to 0.08. Cataract was present in 10 cases with aphakic in two other. B scan demonstrated posterior staphyloma in all. At surgery, strong V-.R adhesion was found from the posterior pole up to the peripheral with TRD. No break was detected. While performing artificial PVD, iatrogenic break was created in 5 eyes. Laser with gas or oil injection was done. Follow up period from 6 months to 3 years.

Results: Retina not attached in 2 eyes at follow up. V.A. of 10 attached retina from CF to 0.1

Conclusion: TRD can occur in high myopic eyes occasionally. Vitrectomy is able to reattach them but with limited visual success.
