Purpose
To compare the clinical outcome between non high myopic and high myopic patients with AcrySof® ReSTOR® diffractive multifocal intraocular lens (IOL).

Methods
This study included 72 eyes of 36 non high myopic patients and 18 eyes of 9 high myopic patients. The main outcome measures, taken at postoperative 1 day, 1 week, 1 month, and 3 months, were uncorrected near and distant visual acuity, target and postoperative refractory errors, and subjective satisfaction.

Results
There was no statistically significant difference in postoperative uncorrected near and distant visual acuity comparing non high myopic and high myopic groups. The mean postoperative refractive errors of two groups each using SRK 2 and SRK/T formulas showed no significant difference compared to the target refractive errors. Overall subjective satisfaction was not significantly different the two groups.

Conclusions
The AcrySof® ReSTOR® diffractive multifocal intraocular lens (IOL) can be effective at improving high myopic patients satisfaction after cataract surgery as well as for obtaining satisfactory uncorrected near and distant visual acuity by predictable postoperative refractive errors using the SRK/T formula.
