A Case of Congenital Glaucoma Using Glaucoma Drainage Device Implantation.

Purpose: To report a case of congenital glaucoma using glaucoma drainage device (GDD) (Baeveldt) implantation. It is a well-known fact that the more premature congenital glaucoma is, the poorer prognosis it has. The first choice of treatment of congenital glaucoma is goniotomy or trabeculotomy. Although trabeculotomy and cyclodialysis were performed three times, these operations were not effective. So we forced to perform a surgery congenital glaucoma using a GDD Implantation.

Methods: A eleven days old male. He had cloudiness of both eyes just after birth and had a medical examination for suspicion of congenital glaucoma. Both corneas swelled and became turbid, and the pupil which can be seen with difficulty, turned over the outside wrongly and dilated in the middle class. Both intraocular pressures (IOP) were 40mmHg. We diagnosed Rieger type congenital glaucoma, and performed trabeculotomy of superior temporal at both eyes in the 11th day of birth. But IOP was out of control. We performed it of inferior temporal side and inferior nasal side in the same way, but it wasn't effective. We added cyclodialysis surgery of the superior temporal quarter side, but it was effective neither. Even if many times glaucoma operation were performed, IOP was not controlled. So glaucoma drainage device (GDD) implantation was selected. The postoperative IOP were 20~30mmHg in right eye, 10 - 25mmHg in left eye.

Results and Conclusions: It seemed GDD implantation as definitive treatment for congenital glaucoma was effective.
