Dacryocystectomy and Dacryorhinostomy in a patient with Lacrimal Sac Tumor

Purpose: To report a case of patient with lacrimal sac tumor and its management dacryocystestomy and dacryorhynostomy

Method: A 50 years old woman came to ophthalmology department, Saiful Anwar Hospital on 30, April 2009 with complaint there was a mass in periorbital region near the nasal area (medial canthal) since 3 years ago. The mass grow slowly progressive accompanied by epiphora, no bloody discharge, pain or inflammation. VA RLE 5/8.5 ph 5/5. There was a mass in medial canthal area, cystic, size diameter 2 cm. On Anel test there was regurgitation. CT scan imaging showed a cystic mass in the lacrimal fossa. The patient was managed with dacryocystectomy and dacryorhinostomy with silicon tube insertion. On the pathology examination tumor material there was a serous cyst. The silicon tube was inserted 6 months after the surgery to maintain the patency and its was evaluated until 3 months later. We evaluated tearing, patency lacrimal excretion (performed anel test), the infection after surgery, and recurrence of the mass.

Result: The evaluation of epiphora, the surgical wound, the patency of lacrimal outflow and recurrence of the mass. After 9 month evaluation the mass had resolved, the wound in medial canthal area was healed and good in cosmetic appearance, the tearing was reduced and relieved patency excretion lacrimal.

Conclusion: Dacryocystectomy and dacryorhinostomy in this patient with lacrimal sac tumor had good result with relieve of tumor mass, no recurrence, relieve epiphora and patency excretion lacrimal drainage.
