The Role Of Modified Osteo-Odonto-Keratoprosthesis (MOOKP)

Purpose: To describe the modified osteodontokeratoprosthesis (MOOKP),originated from Strampelli's OOKP, with the modifications and the innovations made by Falcinelli, to restore sight in corneal blindness not curable by any type of keratoplasty

Methods: First stage: Preparation of the lamina with abstraction of the mono-rooted tooth with root and surrounding bone: the PMMA optic cylinder is glued, in the hole of the lamina and burried for 3 months in subcutaneous pocket. Preparation of the eye: opening of the anterior chamber, complete removal of the iris, lens cryo-extraction, anterior vitrectomy. Intermediate stage: After 1 month covering of the eye surface by buccal mucosa. Final stage: after3 months implant of the lamina on the eye with insertion in the A.C. of the cylinder after corneal trephining, covering with buccal mucosa previously detached. Complications treatment are described.

Results: BEST V.A: 90% of operated eyes obtained a useful V.A between 10/10 and 2/10; 7% obtained ipovision; Only 3% H:M or no improvement. FINALLY V.A. Of 90% results with useful V.A almost 80% maintained the same V.A, with follow up from 10 years to 31 years.

Conclusion: Biological properties Strampelli's osteo-odonto lamina with Falcinelli'ssurgical modifications make of MOOKP a KPro really capable to restore the sight and to maintain it for life, in every cases of blindness caused by any possible corneal scars affecting the anterior surface of the eye, the adnexae, including the lids, up to last of dry eye.
