Objective: To evaluate visual outcome and complications after pars plana vitrectomy and posterior segment intraocular foreign-body removal.

Methods: A case series involving 150 consecutive cases that underwent pars plana vitrectomy and the removal of IOFB from November 1999 to December 2009 was included. A standard three-port pars-plana vitrectomy with or without endolaser, cryotherapy, intraocular gases or silicone oil endotamponade was performed. Initial visual acuity, time between injury and presentation, nature and mechanism of injury, site of entry wound and associated ocular injury, size and location of IOFB were documented. Post surgical complications and visual outcome were assessed.

Results: After at least 6 months of follow up, eyes that achieved vision worse than 3/60 were deemed as poor visual outcome. Poor initial visual acuity, presence of RAPD, macular site of impact, endophthalmitis and development of retinal detachment were identified as factors related to poor visual outcome in IOFB injury.

Conclusion: Most eyes with retained IOFBs maintain useful vision with appropriate treatment.
