After the lateral rectus muscle is hooked with a Stevens and then a Green muscle hook, the lateral rectus muscle is tented to allow placement of a 4-0 silk suture beneath the insertion of the lateral rectus muscle. The needle tip should glide along the inferior surface of the muscle hook to avoid laceration of the lateral rectus muscle or laceration or penetration of the globe.
