Biaxial Phacoemulsification and Acri.Smart Lens Implantation for Hypermature Cataract |

To assess the feasibility, risks, and postoperative outcomes of biaxial phacoemulsification with Acri.smart lens implantation for hypermature cataract cases. Methods: This consecutive prospective study comprised 28 eyes of 28 patients with hypermature cataract . Biaxial phacoemulsificatio was performed through two clear cornea incisions which were made 90 degrees from each other using a 22-gauge disposable needle. Trypan blue was routinely used for the anterior lens capsule staining for its visualization during continuous curvilinear capsulorhexis(CCC) with a 23-gauge micro capsulorhexiser in the absence of a red reflex. Acri.smart lens was injected using a hydraulic cartridge and injector through a enlarged 1.5mm incision. Uncorrected and corrected visual acuity, intraoperative and postoperative complications, preoperative and postoperative endothelial cell count, and incision size were recorded.Results: The mean uncorrected visual acuity improved significantly from 20/800 (0.025±0.025 decimal value) preoperatively to20/30(0.7±0.4 decimal value) 1 month postoperatively(P﹤.000 ). The best corrected visual acuity improved significantly from 10/100 (0.01±0.01) preoperatively to 20/25 (0.8±0.4) 1 month postoperative(P﹤.000). Intraoperative complications included 1 incomplete capsulorhexis and 1 intraoperative corneal edema. Postoperatively, 3 eyes had corneal edema and 2 eye had more than 2+cells and flare. All disappeared by intensive topical and subconjunctival steroids. average endothelial cell lose was 11.6%.
