| [摘 要] 目的 比较不同透明角膜切口位置对白内障超声乳化术后角膜散光及视功能的影响。方法 白内障患者60例（60眼）随机分成两组，A组行上方11点方位透明角膜切口，B组按最大角膜屈光度子午线方位做角膜切口，分别行超声乳化摘除联合人工晶状体植入术，术后1周、1月及3月回访病人，行医学验光及角膜地形图检查。结果 术后1周、1月及3月2组患者之间矫正球镜后最佳视力对比差异无显著性（P﹥0.05）。术后1周A组角膜散光度明显大于B组角膜散光度（P﹤0.05），A组术后1月角膜散光度明显大于术后3月角膜散光度（P﹤0.05），B组术后1周角膜散光度明显大于术前及术后3月（P﹤0.05），术后1月较术前、术后1周及术后3月无显著性差异（P﹥0.05）。结论 根据角膜地形图的角膜散光轴向选择透明角膜切口对角膜散光影响小，早期可获得较好视功能。
[Abstract] Objective To compare the effect of different clear corneal incision position in phacoemulsification on posteroperative astigmatism .Methods 60 eyes of 60 patients were selected and divided into A ( incision on the steepest corneal meridian)and B group ( incision on the superior position ) randomly to accept phacoemulsification and implanted IOLs . Results 1week, 1month and 3month after the operation, the vision corrected with spherical lens was no significant difference between two groups﹙P﹥0.05﹚. 1week, after the operation, the corneal astigmatism in group A was significantly higher than group B﹙P﹤0.05﹚. the corneal astigmatism in group A 1 month postoperatively was significantly higher than in 3 month ﹙P﹤0.05﹚, There were no significant difference in group B between 1 month and 3 month﹙P﹥0.05﹚. Conclusion clear corneal incision on the steepest corneal meridian is a useful way to acquire better visual quality ,the postoperative corneal astigmatism can be reduced significantly. |
