Treatment and Pathologic Analysis of Highly Myopic Strabismus

Objective:To report the pathological changes and effect of surgical treatment for highly myopic strabismus. Methods: We reported one case of myopic strabismus fixus that were treated with a loop myopexy of the lateral rectus ( LR) and superior rectus (SR) muscles. Muscle biopsies of extraocular muscle and biceps brachii were done with routine and histopathologic staining after being frozen in precooled isopentane for cryo-section. Photoelectric microscopy was made. Results: At two months postoperative, she only had 10△ esotropia and 5△ hypertropia of the left eye. Motility gradually improved in all directions. The patient’s findings remained stable throughout 10 months of follow-up. Photoelectric microscopy on extraocular muscles manifested coexistence of atrophy, degeneration and regeneration of muscle fiber. The myofilaments were disarranged. Fibrous tissue and adipose tissue increased between muscle bundle. Electron microscope examination on extraocular muscles revealed that the mitochondria were aggregated. Lipid droplet also increased. Photoelectric microscopy on biceps brachii showed orderly arranged myofilaments. Conclusions:We do not think that myopic strabismus fixus is a systemic disease. Meanwhile we think that when superotemporal herniation of the globe is confirmed by MRI or other images, looping the SR and LR muscles together can restore the herniated globe back into the muscle cone and get good result.
