Characteristics of Sterile vitritis after Moscow Eye Microsurgery Complex Keratoprosthesis

Objective  Retrospective review of consecutive clinical case series to determine the characteristics of sterile vitritis in eyes after the Moscow Eye Microsurgery Complex (MICOF) keratoprosthesis.
Methods  The study took place at Chinese PLA General Hospital. Between April 1, 2000, and May 1, 2010, 96 eyes of 96 patients underwent MICOF keratoprosthesis implantation. Six of these 96 patients developed sterile vitritis. Data especially visual acuity (VA) was collected from the preoperative and postoperative course.
Results  Sterile vitritis occurred in 6 patients, 1 to 30 months postoperatively. All patients presented with sudden, marked decrease in vision with little or no pain. The recovery of vision was complete in 2 to 6 weeks. Case 1 and case 3 were treated like bacterial endophthalmitis. Both of the two patients underwent pars plana vitrectomy as soon as the diagnosis was confirmed. Peribulbar injection of Dexamethasone, Tobramycin, 1% Lidocaine (DG mixture) and antibiotics were used in the first 3 days of the other four patients. Triamcinolone (40mg) and intensive topical steroid therapy were applied then after. The recovery of vision almost to the pre-episode level was seen in case 2, 4, 5 and 6.
Conclusions  Sterile vitritis after MICOF keratoprosthesis would be an immune phenomenon similar to the nature of graft injection. Peribulbar injection of DG mixture, triamcinolone, and intensive topical steroid might be a proper therapy in our series.
