Pupil dynamics after retropupillary implantation of Iris-Claw IOL in aphakia-- A comparative retrospective study between pupillometries after IOL implantations of Iris-Claw IOLs and foldable in-the-bag IOLs.

Purpose  To evaluate the pupil size and mobility at different illuminations with Procyon Pupillometer after retropupillary iris-claw IOL fixation for correcting aphakia.
Material and Method  37 eyes of 26 patients were included in this retrospective comparative study. In group 1, 17 eyes of 16 patients underwent retropupillary iris-claw verisyse  IOL fixation from Feb 2008 to Aug 2010. In group 2, 20 eyes of 10 patients underwent Rayner 630F multifocal fodable IOL in-the-bag implantion after regular phacoemulsification from Feb. 2010 to Dec. 2010. The pupil size were measured at different illuminations (0,04 lux, 0,4 lux and 4 lux) by Procyon 2000 pupillometry on all the included eyes 4-35 months after operation.
Results  The mean pupil size at Scotopic, Mesopic Lo and Mesopic Hi are 4.31mm±0.72mm, 3.93mm±0.65mm, 3.48mm±0.69mm in group 1 and 5,24mm±0,69mm, 4,27mm±0,73mm, and 3,51mm±0,69mm in group 2 respectively. The mean pupil size changes from Scotopic to Mesopic Hi are 0,82mm±0.44mm and 1.73mm±0.31mm in each group. When compared between groups, mean pupil sizes show statistical significant differences only in Scotopic illumination (P<0,001), but not in the other two illuminations (P>0,05). In each group, the pupil contracts when changing from Scotopic to Mesopic Hi (P<0.001), but the pupil size change more in group 2 (P<0.001).
Conclusion  After retropupillary iris-claw IOL implantation, the pupil size doesn’t show significant change compared with foldable in-the-bag IOL implantation at middle or high illumination. However, at dark conditions, the pupil’s dilation is restircted with statistical significant differences. Iris enclevation maybe a reason for the difference. However, we can’t exclude the original existing iris damages due to previous surgery as one of the reasons.
