Analysis of subconjunctive silicone oil after 23G sutureless vitrectomy: A review of 64 cases

PURPOSE The pathogenesis of the subconjunctive silicone oil(SO) associated with 23Gsutureless vitrectomy remains controversial. Through this study, we report and analysis the relevant factors may attribute to subconjunctive silicone oil.
Patients and Methods This was a retrospective single-centre study of 64 patients underwent 23-gauge transconjunctival sutureless pars plana vitrectomy (PPV) with silicone oil tamponade in Wenzhou Eye hospital between 2010 and 2012, follow-up with 1week, 1 month up to 1 year. Slit lame exam demonstrated subconjunctive SO nodule near SO injection site. Application of anterior segment OCT to observe of architectural structure of scleral incision. All subconjunctive SO nodule were removed with syringe needle in outpatient facilities successfully.
RESULT 55 of 64 eyes reported relief symptom. Long duration of operation>2hrs (30.2%), pathologic myopia(25%), uveitis(12%) , diabetics (42.5%), and multiple operations (35.2%) are the leading causes for the subconjunctive SO. The pathology of specimens demonstrated foreign body granuloma.
CONCLUSION Long duration of operation>2hrs, pathologic myopia, uveitis, diabetics, and multiple operations seem to affect the architectural structure of scleral incision which may leading to SO leakage.
