Methods We studied a retrospective interventional case series of 38 eyes in 34 patients with IERM. All patients underwent standard three-port pars plana vitrectomy (PPV) with peeling off of the epiretinal macular membrane.Regular examinations results also were analyzed in pre-operation and post-operation. The multiple factors related with age, course ,preoperative best corrected visual acuity(BCVA) preoperative mean central macular thickness (CMT),internal limit membrane peeling technique,triamcinolone-assisted internal limiting membrane peeling technique were analyzed.A microstructure of the fundus was obtained by spectral domain-opticalcoherence tomography (SD-OCT) to assess the preoperative foveal anatomy and the thickness of the retina. To evaluate the relationship between the integrity of the photoreceptor inner segment/outer segment (IS/OS) junction and preoperative BCVA .Main outcome measures were the images of the foveal structure by SD-OCT and the BCVA measured simultaneously.
Results After a 12.2±13.5 month follow-up, the postoperative BCVA significantly improved in 25 eyes (65.8%), invariable vision was observed in 8 eyes (21.0%) and descended vision was observed in 5 eyes (13.2%).SD-OCT showed that the CMT was (399.900±151.66) μm (256.00-1140.00).The intact IS/OS junction was detected in 18 eyes (47.4%), 20 eyes (52.6%) had disruption of IS/OS junction. There were significant difference of preoperative BCVA (P=0.003), preoperative CMT (P=0.036) and the integrity of the IS/OS junction (P=0.000).
Conclusion The effect of surgery on the treatment of IERM determined by several factors .Thanks to high-resolution and 3D scanning protocols, more information can be gathered. Morphological changes in the retina, such as preoperative CMT and photoreceptor defects, were present in patients with IERM. Estimation of those changes may be an important prognostic factor in cases of epiretinal membranes. And the integrity of the IS/OS junction reading is closely related to patients ' visual acuity.
