Prognostic Value of Ocular Trauma Score in Ocular Firecracker Trauma

PURPOSE: To investigate the incidence and treatment of ocular firecracker trauma. To measure the predictive value of ocular trauma score (OTS) in ocular firecracker trauma.

METHODS: Consecutive selection and retrospective analysis of ocular firecracker trauma. Certain numerical values rendered to the OTS variables (visual acuity, rupture, endophthalmitis, perforating or penetrating injury, retinal detachment and afferent pupillary defect) at presentation were summated and converted into OTS categories. The likelihood of the final visual acuities in each category was calculated. The sensitivity and specificity of OTS were then calculated.

RESULTS54 eyes of 48 cases were enrolled in the study. Male patients accounted for 85.4%. 62.5% patients were younger than 15 years old. Bystanders accounted for 47.9%. Average following up was 18.5±3.6 months. The most common clinical manifestation was ocular penetration or perforation injuries. OTS summation mostly was 66-80 categories. 53.7% patients’ corrected visions at the last follow-up were 20/200 to 20/50. OTS has 100% sensitivity and 100% specificity to predict NLP, LP/HM and 1/200 to 19/200. OTS has 45% sensitivity and 100% specificity to predict 20/200 to 20/50. OTS has 100% sensitivity and 65% specificity to predict ≥20/40.

CONCLUSIONS: Clinical manifestations of ocular firecracker trauma in our city are severe than that in other countries and similar with the domestic reports. OTS has good predictive value for ocular firecracker trauma, which has 100% sensitivity and 100% specificity to predict NLP, LP/HM and 1/200 to 19/200.
