Optical coherence tomography-guided intracameral air injection for treatment of extensive Descemet membrane detachment

Purpose:To report a method of anterior segment optical coherence tomography (OCT) -guided aqueous fluid drainage and intracameral air injection for treatment of Descemet membrane detachment (DMD) in 3 patients after cataract surgery.
Methods:3 cases of extensive stripped Descemet membrane detachment were detected at 4, 15 and 30 days after cataract surgery. Anterior segment morphology and configuration of the detachment were assessed with Visante OCT before treatment. The localization of the incisions was chosen according to OCT data. After draining most of aqueous fluid, filtered air was injected with a 30-gauge needle to form a single gas bubble. OCT was performed to estimate the treatment outcomes.
Results:Total reattachment was achieved in all cases, and no further surgical interference was performed. Corneal clarity was recovered within 1 week after air injection, and the period for total absorption of the air bubble ranged from 4 to 5 days.
Conclusions:Surgical intervention with air injection in the anterior chamber under the guide of OCT resulted in excellent visual acuity restoration in DMD. OCT is valuable in precisely and rapidly evaluating the configuration of DMD for an optimal surgical plan and monitoring the treatment results.
