Outcomes With The Ex-PRESS Mini Shunt (Ex-PRESS) Using Mitomycin C, Ologen with 5-Fluorouracil, and Ologen Alone

Purpose Ologen collagen matrix is a newer biodegradable material that may prevent scar formation and create less avascular bleb, therefore it may prevent severe complications compared to MMC when used in trabeculectomy. No studies have explored efficacy of Ologen in regard to the Ex-PRSS.  This study is to compare the efficacy of the Ex-PRESS with Mitomycin C (MMC), Ologen with 5-Fluorouracil (5-FU) or Ologen collagen matrix alone.
Methods All uncontrolled glaucoma patients who had the Ex-PRESS at University of California San Francisco from 2009 to 2011, a total of 38 eyes of 31 patients, were studied.
Results At 6 months, IOPs were 14.7mmHg, 13.4mmHg, and 21.2mmHg in the MMC, Ologen with 5-FU, and Ologen only group, respectively (p=0.05). At 1 year, IOPs were 12.0mmHg in the MMC group, 12.2mmHg in the Ologen with 5-FU group, and 19.4mmHg in the Ologen only group (p=0.012). Endophthalmitis only occurred in the MMC group.
Conclusions This study demonstrates that Ologen may prevent the severe complications such as endophthalmitis in Ex-PRESS implantation but IOP appears to be poorly controlled at 6-month and 1-year post-surgical course when used alone. However, when Ologen combined with 5-FU, it has equal efficacy as MMC at controlling the IOP up to one year.
