Acute visual loss after injection hyaluronic acid into the facial skin.

Purpose To report two females presented with sudden visual loss of unilateral eye after
receiving a hyaluronic acid (HA) injection into the facial skin for the cosmetic purpose.

Method Review the clinical records of the two patients.
Result Fundus examination and fluorescein angiogram showed multiple embolization at retinal arteries, andthe patchy choroidal filling was demonstrated at fluorescein angiography of the second patient. Inhaled oxygen, ocular massage and intravenous methylprednisolone treatment was given to them too. The disc edema subsided and retinal colour was normalized about 2 weeks later, but there was no improvement in their visual acuity. The HA injected is thought to have entered the facial vessels system and the retrograde migration of the emboli to the ophthalmic artery might have caused the multiple occlusions of the branches of the
ophthalmic artery.

Conclusion We suggest that the HA injection for cosmetic purpose should be cautious to perform.
