光学相干视网膜断层扫描对先天性黄斑区视网膜劈裂的研究

Purpose: To further improve our understanding of congenital juvenile retinoschisis, and to find the value of Optic Coherence Tomography(OCT) in its diagnosis. Methods: The details of the posterior fundus in 20 eyes from 11 patients, who were diagnosed as congenital retinoshisis, went checked thoroughly by OCT. Results: In the OCT results, schisis cavity at the macular was found in all patients, and there were fine vertical retinal elements bridging between the outer and inner wall of the retina. From the original figure and further analysis, we found that the schisis cavity was not at the place between the nerve fiber layer and the gangling cell layer as expected, but much deeper in the middle part of the retina. Conclusion: OCT may have high specificity and sensitivity in the diagnosis of congenital retinoschisis. In congenital juvenile retinoschisis, the schisis cavity at the macular area may located at a deeper place rather than beneath the nerve fiber layer, but further study is required. 目的:为加深对先天性视网膜劈裂的认识和了解光学相干视网膜断层扫描仪(OCT)在其诊断中的价值。方法:对临床诊断明确的11名先天性视网膜劈裂患者的20眼,使用OCT对患者后极部视网膜的细微结构进行观察。结果:在先天性视网膜劈裂患者的OCT图像中,视网膜内有明确的劈裂腔隙,劈裂的内外层之间有组织柱相联。但根据其中图像和对图像进行的分析,我们发现劈裂所在的层次位于视网膜的中层,较通常所认为的神经纤维层要深。结论:OCT在先天性视网膜劈裂的诊断中可能有很高的特异性和敏感性;同时在患者的黄斑部,视网膜劈裂的位置与传统的认识之间可能存在一定的差异,但有关结果需进一步证实。
