Purpose: In order to facilitate finding Schlemm’s canal in trabeculotomy ab externo for congenital infantile glaucoma.
Methods: Surgical procedures include 1) a limbus-based conjunctival flap,
2) 1st transillumination with ocutome illuminator to identify the surgical landmark, 3) a laminar limbus-based trapezoid scleral flap, 4) 2nd transillumination to identify the Schlemm’s canal and trabeculum area, 5) a second deep pentagonal flap, dissecting from supra-ciliary body beneath the first flap, usually including deroofing of Schlemm’s canal, then excising the second flap, 6) finding the Schlemm’s canal and performing trabeculotomy, 7) suturing the scleral flap tightly, then injecting the Healon-GV into subscleral-flap space, and 8) a running suture of the conjunctival flap.
Conclusions: The primary experience indicated that using scattering illumination and deep slerectomy methods in trabeculotomy for congenital glaucoma could facilitate finding the Schlemm’s canal and make the trabeculotomy easily. At the same time, the created space by excising the second flap might also increase the internal drainage through the theoretical uveoscleral flow. |
