目的 探讨虹膜角膜内皮综合征(iridocorneal endothelial syndrome,ICE综合征)合并白内障患者前囊膜的病理改变及其发病机理。方法 对患者行现代白内障囊外摘除术,取术中连续环行撕囊的前囊膜进行光镜、电镜及免疫组化观察。结果 前囊膜光镜下晶状体上皮细胞呈增殖样改变;透射电镜见晶状体上皮细胞增殖伴大量的成纤维样细胞;免疫组化染色角蛋白阴性、波形蛋白、转化生长因子β1(transforming growth factor beta,TGF-β1)、α-平滑肌蛋白(α-smooth muscle actin ,α-SMA)、碱性成纤维生长因子(basic fibroblastic growth factor, bFGF)、白介素-1(interleukin-1 beta,IL-1)均为阳性。结论 ICE综合征合并白内障前囊膜上晶状体上皮细胞呈增殖样改变并向成纤维细胞化生,TGF-β1等细胞因子在晶状体上皮细胞的增殖转化过程中起重要作用。
[Abstract] Objective To study the pathological alteration of anterior capsule of iridocorneal endothelial syndrome with cataract. Methods The anterior capsule was obtained during the extracapsular cataract surgery and was examined by light microscope, transmission electron microscope and immunohistochemical stain. Results The lens epithelial cells proliferate and transform into spindle-shaped myofibroblast cells,which express vimentin, transforming growth factor beta(TGF-β1),α-smooth muscle actin(α-SMA), basic fibroblastic growth factor( bFGF), interleukin-1 beta(IL-1β). Conclusion The lens epithelial cells of iridocorneal endothelial syndrome with cataract proliferate and undergo fibrous metaplasia. Cytokines plays an influential part in the genesis of pathological alteration. |
