| Purpose: This is a case report about a 73-years-old lady who presented 7 years ofter Cataract surgery with a marked reduction in her distance BCDVA.
Setting: IVCRC, Dept. of Ophthalmology
Methods: The BCDVA was -7,5/-2,0/60Grad: 0,8p. Her old prescription on the left eye was: -2,5/ -1,0/90Grad. Anterior segment was analysed using Visante OCT and VuNax UBM. Results: The anterior segment left presented an ACD of 2mm, whereas right it was 3.69mm. The entire Iris diaphragma shifted one milimeter forward.
Results: The case showed a socalled “relative anterior Mikrophthalmus (RAM). There is a cilio-vitreo-pseudophakic Blockage Syndrome often seen with RAM. We performed a parsplana vitrectomy 2 days later the ACD was 3,66mm and visual acuity and refraction - were 2,5/-0,5/15Grad: 0,8p.
