We compare the wavefront aberration and corneal topography measurement and their respective clinical importance. Measurement accuracy, dynamic range, repeatability, spatial resolution, spatial coverage are discussed and compared for both corneal topography and wavefront measurement. We discuss asphericity, spherical aberrations using Zernike analysis and simulation of point spread function for various shape of corneal surfaces. We then discuss a topo-driven, 3D elevation data based, prolate shape and ellipsoidal model based, volumetric ablation algorithm. We emphasize the clinical aspects of the approach, particularly during patient pre-op measurement and treatment, critical to both understanding the algorithm and in utilizing the algorithm. We also discuss the possible cause of the improper ablation, particularly that related to improper use of eye tracking and its consequences. We discuss a tissue saving algorithm in optimizing the post-op corneal shape and axis to effectively address the complicated cases such as enhancement for de-centered ablation and other irregular corneas. We introduce the concept of anchor axis, in-situ reference identification and patient alignment and its clinical applications. We discuss a new concept with our sololase system –an all laser PRK procedure that eliminate the need for any mechanical device. Numerous clinical examples of complex cases of enhancement from multiple hospitals using the technology are given to show the effectiveness of the technology in producing significant clinical outcomes including and particularly for irregular corneas and other complex corneas.
