光动力疗法联合玻璃体腔Avastin注射治疗视乳头毛细血管瘤一例

【摘要】 目的：对光动力疗法联合玻璃体腔Avastin注射治疗对1例von Hipple－Lindau病中视乳头毛细血管瘤的疗效观察；方法：经眼底荧光造影以及B超和颅脑CT检查确诊为von Hipple－Lindau病伴视乳头毛细血管瘤，最佳矫正视力为0.1，FFA示视乳头荧光渗漏面积为11平方毫米。先予以光动力治疗（能量50j/c㎡,时间83s，光斑直径5200u），一周后复查，视力下降至0.04，视乳头周围网膜出血，水肿。2周后行玻璃体腔注射Avastine 0.1ml，每次治疗间隔4周，现已连续6次。结果：现视力恢复至0.8，FFA示出血已基本吸收，瘤体渗漏面积明显减小，未见其他明显并发症发生。讨论：光动力疗法联合玻璃体腔注射Avastin对视乳头毛细血管瘤是相对安全有效的治疗方法，可使瘤体缩小提高视力，但可能引起出血等并发症发生。

Comparison of Photodynamic therapy and intravitreous injection with Avastin for Capillary angioma of the optic paillary
【Abstractive】Objective:To observe the efficacy of photodynamic therapy(PDT) combined with intravitreaous injection with Avastin in one patient of capillary angioma of the opic papillary.Methods:One patient,the visual acuity was 20/200,were diagonosed as Lindau-von Hipple disease with capillary angioma of the opic papillary by B scan and Cranial Computed Tomography.At first,PDT was performed with laser parameter 50J/C㎡,83s,5.2mm in size.After1 week the visual acuity was decreased to 20/500. Retinal was hemorrhage and edema.After 2 weeks intravitreous injection was performed wih Avastin(0.1ml) once 4 weeks.Results:Visual acuity is 20/60 now. Retinal hemorrhage have been absorpted,and fluorescence leakage was deflated by FFA.No complication take place.Conclusion:intravitreous injection with Avastin is more effective and safe to reduce capillary angioma of the opic papillary size and improve visual acuity than PDT.
