Pentacam前房分析仪评估白内障超声乳化术后相关的角膜损伤

【摘要】 目的 评价Pentacam前房分析仪对白内障超声乳化手术后角膜内皮细胞损伤评估的有效性。方法 对68例老年性白内障行晶状体超声乳化吸出人工晶状体植入术,术前及术后1月分别使用非接触角膜内皮镜观察中部角膜内皮细胞计数变化,以Pentacam测量中心角膜厚度(corneal thickness;CT)及3mm, 5mm, 7mm不同直径分区的角膜容积(corneal volume)变化,并进行组间比较;探讨两种检查方法的相关性;并根据术前不同角膜内皮细胞密度分组,比较术后不同组之间角膜容积的改变。结果 术后1个月,使用角膜内皮镜检查的角膜内皮细胞计数较之术前,差异无统计学意义(P>0.05);使用Pentacam在术后1个月测量角膜容积较术前增大,差异有统计学意义(P<0.05);测量中心角膜厚度较术前增大,差异无统计学意义(P>0.05);不同直径分区角膜容积较术前均有所增大,差异有统计学意义(P<0.05),无论术前与术后,组间比较差异均有显著的统计学意义(P<0.0001);术前不同角膜内皮细胞密度与术后1月角膜容积值呈显著负相关(r=- 0.658,P< 0.001) 。 结论 Pentacam前房分析仪可以更有效的评估白内障超声乳化手术对整体角膜的损伤情况,参数测定为临床评估预后提供重要参考。

Pentacam corneal volume in the assessment of corneal endothelial damage after phacoemulsification

【Abstract】Purpose To evaluate the effection of the Pentacam corneal volume in the assessment of corneal endothelial damage caused by phacoemulsification and aspiration(PEA). Methods PEA was performed in 68 patients by the same surgeon. Corneal endothelial cell changes were measured using a specular microscope and Pentacam before and one month after surgery. Corneal thickness and 3mm, 5mm, 7mm of corneal volume were determined quantatively by using Pentacam before and one month after surgery, to study the relationship between corneal volume and the corneal endothelial change in PEA. Result Using the specular microscope measurement, the endothelial cells count was 2667.63±272.85/mm2 preoperatively, after operation one month the count was 2545.56±298.64/mm2, there were no significant different(P>0.05). Using the Pentacam evaluating, the corneal volume was 59.48±3.87 mm3 preoperatively, after operation one month the volume was 61.68±4.25 mm3, there were statistic different(P<0.05). Comparison the two methods, the latter was better to evaluate the corneal change. A significant negative correlation existed between the preoperative endothelial cells count and corneal volume one month after surgery (P<0.001). Conclusions Pentacam can evaluate the whole corneal injures caused the phacoemulsification and aspiration effectely.
