目的 比较透明晶状体摘除折叠人工晶体植入术和准分子激光原位角膜磨镶术(Lasik)治疗1200D以上超高度近视患者的临床效果。方法 随机选择87例167眼超高度近视并分为两组:A组:透明晶体摘除组,41例(79眼),采用经透明角膜切口超声乳化方式摘除透明晶状体联合植入Alcon公司Acrosoff折叠人工晶体;B组:Lasik手术组, 46例(88眼),采用Moria显微角膜板层刀制瓣和Nidek-EC5000准分子激光机扫描完成Lasik术。观察术前及术后1周裸眼视力、术后3月远、近视力的屈光状态及术后并发症。结果 术后1周两组的裸眼远视力都比术前有明显增加,但A组远视力增加的幅度明显优于B组(x2 =34.457 P<0.01﹞。术后3月A组的屈光状态与B组相比:矫正远视力的球镜度A组明显低于B组(球镜t=2.5287 P<0.05);柱镜t=2.018 P<0.050),矫正近视力B组比A组球镜屈光度差异有高度显著性(t=3.9354 P<0.01)。随访:A组均未发现眩光、夜视力下降、视网膜脱离、复视及后发障等并发症。结论 经透明角膜行晶状体摘除联合折叠人工晶体植入术治疗超高度近视能获得比Lasik更令人满意的视觉效果,而且无Lasik术后可能导致的近、远期并发症的发生。

Comparison of the outcomes of trasparent lens extraction with fold intraocular lens implantation and Lasik for super high myopia

[Abstract] Objective To comparison of both surgery of trasparent lens extraction with fold intraocular lens implantation(TEFIOL) and Lasik for super high myopia. Methods A series of eyes of 167(87 cases) super high myopia patients were divided into two groups: Group A :trasparent lens extraction :79eye (41 patient) had extracted trasparent lens with fold intraocular lens implantation to capsular. Group B LASIK:88eye(46cases) had treated by Moria-turbokeratome and Nidek -EC5000 excime laser. All of cases were analyzed with far-near UCVA,BCVA and refractive power preoperatively and at 1week 、3month postoperatively. Results Distance UCVA of both groups increased more obviously than that of preoperation at 1week, but difference in both groups has statistical signficance(x2=34.457 P<0.01). Refractive power at 3month postoperatively in group A increased more obviously than that in group B(sphere t=2.5287,P<0.05;cylinder t=2.018 P<0.05). near BCVA in both groups has a very statistical significanse(t=3.9354 P<0.01). Both groups were no severe complications during operation and postoperatively. Conclusion The visional recovery effect for the surgery of used clear lens extraction with fold intraocular lens implantation is mor satisfactory than LASIK for super high myopia and complications are less.
