Astigmatism correction is becoming more important for modern cataract refractive surgery, especially for patients with premium intraocular lens (IOL) implantation. We have known well and done a lot to correct astigmatism intraoperatively, such as use of on axis incision, corneal relaxation incision, and toric IOLs. In comparison with intraoperative correction, we usually didn’t pay enough attention to postoperative residual astigmatism which is highly related to patient’s satisfaction. Although excimer corneal laser correction still remains the most accurate treatment for postoperative refraction errors, it is not practically achievable for every cataract surgeon. Peripheral Corneal relaxation incision is simple and easy to correct residual astigmatism but has the shortage of poor predictability. Use slit lamp to assist in doing peripheral corneal relaxation incision will make it more convenient and doable under office setting instead of operation room setting. With new approach of peripheral corneal relaxation incision we could easily adjust postoperative residual astigmatism step by step and thus effectively raise up satisfaction for our patients.
