恒河猴有晶体眼前房型人工晶体植入的实验研究

恒河猴有晶体眼前房型人工晶体植入的实验研究 中山大学中山眼科中心 谭钢,钟兴武,普蔼君,聂昊辉 【摘要】 目的 观察前房型人工晶体植入对恒河猴眼前节的影响,评价有晶体眼前房型人工晶体植入术的安全性。 方法 健康恒河猴7只,一眼植入前房型人工晶体,另一眼为对照眼。术前及术后不同时间点分别进行共焦显微镜、OrbscanⅡ角膜地形图、屈光状态和眼压检查,并于术后6个月采用眼前节成像分析系统对人工晶体及其周围进行观察,同时进行组织病理学检查。结果 7只猴均成功植入前房;型人工晶体,角膜后表面至人工晶体前表面平均距离为(2454±170)µm,人工晶体后表面至晶状体前囊膜平均距离为(410±114)µm;组织病理学观察见手术眼和对照眼小梁网、前房角、虹膜及睫状肌结构均正常,但其内均可见少量淋巴细胞浸润,未见中性粒细胞浸润;手术眼术前角膜内皮细胞数平均为(2875±103)个/mm²,术后6个月为(2351±451)个/mm²,差异有统计学意义(t=5.42,p=0.0002),但术前和术后角膜前、后基质细胞数差异均无统计学意义(p>0.05)。结论 有晶状体眼前房型人工晶体植入对角膜、前房及前房角、晶状体的影响轻微,植入人工晶体没有产生非特异性炎症反应,但术后角膜内皮细胞数较术前减少。 【关键词】 晶体,人工; 晶体植入,眼内; 动物,实验 基金项目:霍英东青年教师基金(91043), 广东省科技计划项目(2003C32707), 广东省自然科学基金重点项目(04105323),教育部“新世纪优秀人才支持计划”(NCET-04-0812). The experimental study of the implantation of phakic anterior chamber intraocular lens in rhesus monkeys Tan Gang, Zhong Xing-wu, Pu Ai-jun, Nie Hao-hui Zhongshan Ophthalmic Center, Sun Yat-sen University, Guangzhou 510060, China. 【Abstract】 Objective To investigate the influence of phakic intraocular lens on anterior segment in rhesus monkeys and to evaluate the safety of implantation of phakic anterior chamber intraocular lens. Methods Phakic anterior chamber intraocular lens was implanted into one eye of the seven healthy monkeys, while the other eye was used as the control eyes. The changes of anterior segment and refractive status were assessed periodically by Anterior segment analysis system, Confocal Microscope、histopathology examination, Orbscan Ⅱ system, retinoscopy, and A-scan ultrasonography. Results Phakic anterior chamber intraocular lenses were successfully implanted in all seven monkeys, and the average distance from the corneal endothelium to the anterior surface of the intraocular lens was 2454µm(2454±170µm). And the average distance from the posterior surface of the intraocular lens to anterior capsule of the crystalline lens was 410 µm(410±114µm). Histopathology showed that TM、the anterior chamber angle、iris and ciliary body were structurally normal, with few lymphocytes infiltrated both in the experimental eyes and control eyes. Corneal endothelium cells of the experimental eyes in preoperative examination and six months after operation were statistically different (2875±103 cells/mm² vs 2351±451 cells/mm², t=5.42, p=0.0002). There was no statistical difference in keratocytes (p>0.05). Conclusion The implantation of phakic anterior chamber intraocular lens had little influence on cornea, anterior chamber, anterior chamber angle, and crystalline lens and caused slight non-specificity inflammatory reaction.
