The appearance of the macular damage immediate after an accidental Nd:YAG laser injury was documented by fundus photography. The clinical course of the lamellar hole formation, the full-thickness hole development and the closure of hole with retinal pigment epithelium metaplasia after surgery were followed constantly by optical coherence tomography. Although the hole finally closed following the standard vitrectomy, the patient did not regain his vision because of the severe damage to the retina and retinal pigment epithelium.
