流程化管理对准分子激光手术质量的作用

【摘要】
目的:在准分子激光手术中引入流程化管理。有效统一患者术前宣教咨询,消除患者对手术的疑虑;有效筛查术前禁忌症,预防手术风险;有效防范术中倂发症,手术操作规范化;有效完善术后服务,显著提高患者满意度。
方法:按中华医学会眼科学会角膜病组制定的《LASIK治疗屈光不正技术规范》拟定术前检查流程、手术流程、术后复查流程;医护人员个体和人数不变,随机设流程化组非流化各200例作为对照;对以下方面进行比较:1、检查效率;2、手术人数在检查人数中的百分比;3、手术效率;4、术后视力;5、患者满意度(咨询解答,服务质量,术后效果)。
结果:流程化组术前检查每人次需1小时、非流程化组需1.5小时;流程化组手术患者占检查患者的70%、非流程化组仅占40%;手术时长不变,流程化组手术量为63人次、非流程化组仅为30人次;术后第一天复查视力≥1.0的,流程化组占90%,非流程化为80%;从咨询解答、服务质量、术后疗效三方面对患者满意度进行调查的结果:流程化组均为好、非流程化组除术后效果为好以外,其它则为不好。
结论:流程化组术前检查所需时间比非流程化组减少0.5小时;流程化组手术患者占检查患者比率的百分比非流程化组多30%;手术时长不变,流程化手术人数比非流程化组增加33人;术后第一天视力检查≥1.0,流程化组比非流程化组多10%;流程化的满意度询问调查中的三项均为好,非流程化组除术后效果为好,其余两项都为不好。

Effect of Workflow Management on the Quality of LASIK

[Abstract] Objective: Introduce workflow management into Lasik. The effective workflow management include: unifying preop-consultation and medical education to eliminate patients’ worries, screening out contraindications to prevent operation risk, preventing complications and standardizing the operation, perfecting postoperative service and improving patients’ satisfaction.
Methods: Plan the preop-exam flow, operation flow and postop-exam flow according to “the technology standard of Lasik vision correction”—established by Corneal Diseases Group of China Ophthalmology Association. With same medical individual and its number, divide patients into two groups, flow group and non-flow group, each group has 200 patients. Comparative study include:1. exam efficiency 2. operation percentage 3. surgery efficiency 4. postoperative UCVA 5. patients’ satisfaction(in consultation, service quality, postoperative effect)
Results: Each patient from flow group need 1 hour in exam and from non-flow group need 1.5 hour. 70% of patients from flow group will take the operation and only 40% from non-flow group will do. With the same operation time, 63 person-time from flow group has performed Lasik and 30 person-time from non-flow group has done. In postop-exam, 90% patients from flow group attained UCVA≥1.0 and 80% from non-flow group did. For patients’ satisfaction(in consultation, service quality, postoperative effect): patients from flow group are all satisfied with above three aspects and patients from non-flow group are only satisfied with their postoperative effect.
Conclusion: Comparative study between flow group and non-flow group showed that : flow group saved 0.5h in exam and increased 30% in the operation percentage, flow group increased 33 persons with the same operation time, flow group increased 10% in cases whose UCVA≥1.0. In satisfaction, three items are all good in flow group and only one aspect(postop effect) is good in non-flow group.
