角膜形态与LASIK术中角膜瓣形态的相关研究

摘要 目的:探讨平均中央角膜曲率、角膜平均直径、制作的角膜瓣平均直径和角膜瓣蒂长度的相互关系。方法:对300例600只眼近视患者手术前角膜地形图测量平均中央角膜曲率,测量角膜平均直径。采用Nidek MK-2000型自动角膜板层刀,负压环选择8.5mm角膜瓣直径,制作角膜瓣后测量角膜瓣平均直径和角膜瓣蒂的长度。结果:得出直线回归方程:角膜瓣平均直径=2.77+0.14中央角膜平均曲率,角膜瓣蒂长度=0.28平均中央角膜曲率-6.63,角膜瓣蒂长度=1.56角膜瓣平均直径-8.11。角膜平均直径与角膜瓣平均直径无直线关系,角膜平均直径与角膜瓣蒂长度无直线关系。结论:平均中央角膜曲率越高,角膜瓣的平均直径越大,角膜瓣蒂长度越长。角膜平均直径与角膜瓣平均直径、角膜瓣蒂长度无直线关系。角膜瓣平均直径越小、角膜瓣蒂越短,角膜瓣稳定性越差,发生游离角膜瓣的可能性越大。关键词:准分子激光角膜原位磨镶术 角膜地形图 角膜瓣
The correlation analysis between corneal form and corneal flap in LASIK

Objective: to study the correlations between the average central corneal curvature, the average corneal diameter,the average diameter of corneal flap and the length of the pedicel of the flap. Method: We have measured the central average corneal curvature and average corneal diameter about 300 patients (600 eyes) before the operation. We have also measured the average diameter of the flap and the length of the pedicel in LASIK operation. Results: By adopting linear regression statistics, we have acquired regression equations as following: the average diameter of corneal flap =2.77+0.14 central average cornea curvature; the length of the pedicel of corneal flap =0.28 average central cornea curvature-6.63; the length of the pedicel of corneal flap =1.56the average diameter of corneal flap -8.11.The average cornea diameter and the average diameter of the flap, the average cornea diameter and the length of the pedicel have no straight-line correlation respectively. Conclusion: The average central cornea curvature is in direct proportion to the average diameter of the flap and the pedicel. If the average diameter of the flap is shorter, so will the pedicel be, the flap will be less stable and there will be more probability that the free flap appears. The surgeons should pay more attention to the correlation between the examining results by corneal topography and the flap form so that complications of the flap in LASIK can be reduced.
