TRIPLE  CATARACT  PROCEDURE

Objective:The triple procedure (simultaneous penetrating keratoplasty [PK], extracapsular cataract extraction [CE] and implantation of a posterior chamber intraocular lens [PCIOL]) was introduced in the mid-seventies. We used the surgical choice for 16 patients with corneal disease and cataract. METHODS: The classical triple procedure were preformed in 16 eyes from July 2001 to December 2004. Of 16 patients, 11 were men and 5 were women. Mean age of these patients was 48.5+/-5.1 years (range, 28-75 years). Reasons for keratoplasty were herpetic or traumatic corneal scarring with cataract (6 cases , 31.2 %), Fuchs' corneal endothelial dystrophy (1 cases, 6.2 %), keratoconus (2 cases, 12.5 %), pseudophakic or aphakic bullous keratopathy (4 cases, 25 %), corneal endothelial decompensation due to other reasons (2 cases, 12.5 %) and regraft (1 cases, 6.25 %).We describe a triple procedure combining corneal transplantation, cataract extraction, and intraocular lens implantation in which the continuous curvilinear capsulorhexis (CCC) is performed in a closed system when corneal transparency is sufficient or using an open-sky method when corneal transparency is poor. RESULTS: The average postoperative follow-up was 23.7+/-17.6 months (range 6 to 48 months); at which time15 (93.8%)of the grafts remained clear. At last follow-up, eyes with good preoperative visual potential, 10cases(62.5 %) achieved a best corrected visual acuity of 20/40 or better. The intraoperative complications included vitreous upthrust in 1 (6.25%) case and posterior capsular dehiscence in 1 case. (6.25%). The most common early postoperative complications were increased intraocular pressure in 3 (18.8 %) and increased anterior chamber reaction in 4 (25%) cases. Posterior capsular opacification was seen in 1 case, graft reaction in none. Central corneal endothelial cell density were 2214+/-175 cells/mm2 . .CONCLUSIONS: The triple procedure is a safe surgical procedure with good graft clarity , reasonable visual recovery, high success rate, rapid visual rehabilitation. For this reason, we consider the TRIPLE procedure as the method of choice for combined lens and corneal opacities.
